Abstract

”Active for Health” is a robust physical activity (PA) and sport programme linking rehabilitation to community PA for seven long term conditions. This poster discusses the Active for Health process and outlines the evaluation methods. Some interim headline outcomes have been presented including; health service utilization savings, quality of life changes and self reported physical activity.
